Acne scars start with the formation of acne. This problem begins when the skin is inundated with sebum (oil) that is produced by the sebaceous glands to moisturize and protect the skin. Outside factors to sebum production include hormone imbalance and high stress levels.

With elevated sebum secretion, the fragile cells that line the follicle can be damaged as the sebum pours through the canals to the surface of the skin. Additionally, other factors can make the sebum even more damaging to the skin. These factors include a deficiency in essential fatty acids, which keep the sebum oil from hardening, and acne causing bacteria.

The change in sebum more easily clogs pores, trapping debris, and causing the bacteria naturally found within the skin to multiply. The body's immune response to fight off these foreign bodies and protect the skin includes an inflammatory response that actually wounds the skin matrix. In their fervent efforts to dispose of these foreign bodies, the immune cells can actually destroy healthy tissue surrounding the damaged cells and bacteria. Furthermore, the irritation to the dermal layer also affects regular collagen production and can ultimately increase the visibility of the resulting scar.

Body acne scars can either be hypertrophic or atrophic. Hypertrophic refers to those scars that protrude from the skin, including keloid scars that, unlike typical hypertrophic scars, continue to expand in size.

These scars are typically found on the chest or back. Atrophic scars, those that are lower than the regular skin level are the types of scars more commonly resulting from facial acne. Ice pick, box car and rolling scars are bound down because the injury is found at the dermal layer, also making these types of scars very difficult to remove.

Facial Acne Scar Treatment Options

To rid facial acne scars, normal scar procedures won’t cut it. Injecting steroids (which have the action of flattening scars with too much collagen) will not decrease the visibility of these types of scars. In fact, it is quite difficult to bring depressed skin up to its original level.

Laser resurfacing for facial acne scars uses non-ablative lasers on the skin, sending quick pulses of high energy light. Converting into heat, the laser vaporizes the layers of skin away. However, you don't simply laser face acne scars away. Scar tissue is eradicated and fresh skin growth is encouraged to replace the tissue that was removed. For a precise, safe method, laser skin resurfacing acne scars causes little outside damage to surrounding skin. For these reasons, it is often the preferred method for treating scars on the face.

There are other resurfacing options but are not as effective. Using dermabrasion on acne scars uses the same idea of regenerating the skin, however, it is less precise. Dermabrasion entails a high speed rotary instrument with abrasive fixtures to remove the layers of skin. Chemical peels on facial acne scars are similar, using a chemical solution to blister the skin and eventually peel off. Both of these resurfacing techniques are used on the entire face and have long recovery times.

Facial surgery on acne scars might be a good option for those that are tied down. Subcision detaches the lower levels of skin so it is less depressed. Punch techniques are mostly used for ice pick and other deep scars. These techniques that remove the tissue with the contained scar include punch excision, punch excision with skin grafting and punch elevation. Depending on the procedure and type of scar, the suture or replacement of remaining tissue follows.

For a temporary solution, dermal fillers can be used to treat facial acne scars as they raise lower areas but must be repeated to retain results.
